How Marex responded

A Human Reliability Assessment forms part of the Safety Critical Task Analysis (SCTA).

The SCTA process is what helps organisations that operate in major accident-hazard industries - such as oil & gas, maritime, nuclear and aviation - systematically analyse the role that human factors play in their operations.

The outcome of safety critical tasks is significantly shaped by human performance.

In a nutshell, the Human Reliability Assessment identifies potential failure scenarios, it enhances operational performance, increases operational safety and identifies operational improvement initiatives.

It gives the client assurance of compliance and goes “beyond peace of mind”.

Analysis process

From a major accident prevention standpoint, there is very high value in breaking down tasks in a hierarchical format and analysing each one from a human performance perspective.

Carried out under Marex’s expert guidance with the involvement of frontline workers from Well-Safe Solutions, this process helps in identifying and recording:

all potential human failure modes and their respective consequences for each task step
existing risk control or recovery measures in place
human performance shaping factors that could contribute to the success or failure of the task steps
improvement opportunities (that is, can we eliminate the human failure potential using engineering controls? Can the task be redesigned? Can the procedures be improved?)

The results
